CSMA/CA - How does the carrier sense mechanism work

It works quite well if you get the RF side sorted. I try to use channels as far away as possible. The separation distance as I mentioned is very reliant on the type and quality of the antenna ie back lobes to the radiation pattern. If there is a minimal back scatter then the antennas can be located closer together. A typical example of mine would be approximately 1.5m.
The throughput is pretty consistent with using two separate AP's, but dependent on the type of traffic being seen.
